All incoming new students and ALL student athletes are required to have a physical exam each year prior to participation on a Gadsden ISD athletic team.

-Students cannot try out, practice, or play for Gadsden ISD athletic team until all athletic healthcare paperwork is uploaded to the athletic trainer.
-No exceptions will be made to this rule..
-Please keep copies of all paperwork for your records.
-When submitting paperwork electronically, the athletic trainer must review and accept it in the Rank One system. This will be done periodically (NOT DAILY), so if you need to access your child’s forms, please understand there may be some delay until the AT is able to review and accept forms.

-Complete and sign electronically vie Rank One Sport under the Electronic Participation Forms tab.

Medical History/Medical Eligibility Form
- Under the Download/Print tab access the Medical History and Medical Eligibility Form. You may fill out the Medical History Form online before printing. Once you have completed the Medical History form print both documents and take them with you to your child's physical appointment.

- After the Medical Eligibility Form is completed, you will need to upload a copy of the Medical Eligibility Form to the Medical Eligibility Upload Form (found under the Electronic Participation Forms Tab).

NFHS Concussion Course:
-Click on the link below to take the NFHS Concussion Course:

- Then under the Electronic Participation Forms, upload the certificate of completion.

Proof of Insurance:
-Provide the required insurance information.
-Then under the Electronic Participation Forms scan and upload a copy of your insurance card..

--Complete and sign electronically vie Rank One Sport under the Electronic Participation Forms tab.
